These Terms of Use (“Terms”) govern your use of the website at revivalfunctionalmedicine.com (the “Site”), operated by Revival Functional Medicine ( “we,” “us,” or “our”). By using the Site, you agree to these Terms. If you do not agree, do not use the Site.

1. Medical disclaimer

The Site is for general educational and informational purposes only. It does not provide medical advice. Reading the Site, contacting us through it, or booking a consultation does not create a provider-patient relationship. That relationship begins only when you become an established patient through our formal intake and consent process.

Nothing on the Site is a substitute for advice from your own licensed healthcare provider. Always talk to your provider before you start, stop, or change any treatment, medication, supplement, or health routine. Never delay or ignore medical advice because of something you read here.

2. No guarantee of results

3. Where our services are available

Our clinical services are available only to individuals located in states where our provider is licensed to practice, currently Colorado. Nothing on the Site is an offer to provide care where our provider is not licensed.

4. Supplements and third-party products

The Site may link to supplements or other products sold by third parties. We may have a financial relationship with those sellers. We may earn a commission when you purchase through our affiliate links. We are not responsible for third-party products, and their sale is governed by the seller’s own terms.

Appointments booked through the Site are scheduled and managed through our third-party platform, Practice Better. Your use of that platform, and any payment, cancellation, or refund terms, are covered by the agreements and policies we provide during booking and intake.

To the fullest extent the law allows, we are not liable for any indirect, incidental, or consequential damages that arise from your use of the Site or your reliance on its content. Your use of the Site is at your own risk.
